Why not forget the ugly word "item" altogether? Then the problem evaporates
and you can simply say:
Are Alan and Nicola still in a relationship?
or
Are they still in a relationship?
Questions like:
"Are Adam and she still an item?" or "Are she
and Adam still an item?"
are rather ungainly and sound awkward.
