I think most developers use a PC together with a cross-compiler (such as
codewarrior; http://www.codewarrior.com/MW/Develop/Games/Default.htm) to
generate Xbox or PS code. They then either burn the game onto a cd-rw/dvd or
they ftp the game to the console via a network cable. Perhaps developers are
able to debug remotely (via the same network cable) as well. In any way,
they must have a modified console to be able to run unsigned code.
