The extra monitor needs to be set up thru the video card software
interface or in control panel (for win7 "set up computer to use
multiple monitors").

You'll usually have several options of how to set them up.  Primarily
the choices are:

1. Duplicate output on both monitors
2. Extended desktop where the second monitor (the hdtv) represents
additional desktop space.   This works nicely if you are going to be
occasionally using other programs on the computer and you'd rather not
have them interrupt the movie on the hdtv.

A useful utility to look for is Ultramon.  It adds controls to all
your windows allowing you to switch which monitor they are displayed
on with just a click on the control next to the minimize control.  So
e.g. if you're viewing a youtube video on the browser, you can click
on the browser window and send it to the hdtv, then go fullscreen with
it.
