I used it.

My scan is from the same source. It's also missing the left edge,
which is pretty stupid for a scan of a table. I might be able to
reconstruct the data as it looks pretty familiar. I'll use the index.
:)
Shield proficiency is cut off on the left. It should read
"This is the maximum number of times in one combat round that the
shield can be used to protect a character."
From PHBOR1 TSR02149 Players Option Combat and Tactics
Top left. The mising text under Battle Tactics in the
first column is
Shield Wall
Mounted Charge
Archery from Horseback
(half-move)
Archery from Horseback
(full-move)
The Psionic Combat chart is from
PHBOR2 TSR02154 Player's Option Skills & Powers
table 76, page 148
It can probably be cut and pasted.
For the knockdowns table it probably reads something like this:
"**Modifiers are applied to base weapon knockdown dice. For
example, a bugbear wielding a morningstar has a knockdown die
of d12 against humans, as a morningstar is a d10 weapon and a
bugbear is one size larger than a human."
