What you are asking is maybe something you
already know of but not "how to", yes?
Essentially you "break up" the document into
pages and edit each page then join them
together again.
Make sense?
How it is done depends on the abilities of the
software used. PDF is "postscript" (ps) so
many a tool will do what you ask.
I would need some time to research this.
Maybe it is more efficient for you to start
without me:-)
